At the Hebrew Rehabilitation Center, we have a centralized approach to the study of falls and fall prevention. All of our patients are assessed for fall risk upon admission and a plan of care is initiated immediately. We enter our information monthly, track and trend for issues and work with staff to prevent harm or injury to our patients.
